Please fill out order forms as completely as possible.
In the personal information section, make sure to include phone number and e-mail address. We may have to contact you if questions arise.
Fill out the section for the type of work you will be submitting. (ie. Slides to DVD)
1. Each order includes one DVD. If you would like to purchase additional copies please indicate the number of copies you would like to receive.
2. Fill out the personalized title to appear on the DVD label. (box at the bottom of the order form) This will appear on the DVD sleeve and disk. We are not responsible for misspelling so please write legibly.
3. If you order a slide show please number slides in the order to be scanned. No need to remove slides from their carousel. We will credit you two dollars per carousel that you turn into recycling with us.
4. Place slides with complete order form. Be sure to seal box well.
5. Now your order(s) is ready to be shipped. You may ship your order(s) to our lab by any method you choose.

Inbound Shipping: Customers may ship inbound orders to Denevi Digital Imaging via the carrier of their choice. Please be certain to have your information on both the inside and outside of your package.
Outbound Shipping: Denevi Digital Imaging offers Fed-Ex ground, Fed-Ex 3 Day Express Saver, and Fed-Ex Overnight. Shipping costs are calculated based on Net billing amount. See shipping rate calculator.
